I like everyone's suggestions, but I didn't read anyone stating that you
might have a rootkit installed, which could account for your
anti-{malware|virus} software not seeing anything.

Check out RootkitRevealer -
http://www.sysinternals.com/utilities/rootkitrevealer.html - and/or
Blacklight - http://www.f-secure.com/blacklight/rootkit.shtml. Both are the
two rootkit detectors which come to mind.
