Redundancy, money, and strict control mechanisms. Most of the
customers I have ever dealt with when told the price of a 5 or 6 nines
operation have decided that they were happy with 2 nines :). This was
in the case where the customer originally wanted that the webpage was
available around the world no matter if their home office was down
etc. The price quote of getting Akamai/etc to front end the static
parts was more than they could afford :). However that is the small
business owner. The larger ones have the money to start getting quotes
from people who do this and will put in the structures (strict ITIL
methodologies for change control).

Depending on the internal operations of the site, it can take months
to get into a 5/6 nines place as you have to go through continual
improvement on the methods of updating webpages, code, backends etc.
And then having the customer read and go over just the wiki-page

they really need/afford that height of precision.
