Any well configured firewall would be blocking outgoing SMTP connections to any address except for the valid mail server.. stopping bugbear and similar viruses with their own mailing code in their tracks....

Ummm, I though most, if not all mass mailers used the SMTP server written in the registry(HKEY_CURRENT_USER\Software\Microsoft\Internet Account\Manager\Accounts\00000001) by (outlook?).
