Many people (not all) who use Outlook don't pay attention to file extensions when opening attachments, plus aren't aware of things like VBScript.

It's the way PC software is evolving - it's made too easy, too many things are automated without the user's knowledge, and suddenly you can send executable code or script in e-mails that can do goodness knows what on the recipient's machine.
