The PSTN protocols require a CPN in some call classes. In that case some of the reserved unassigned numbers could be used to clearly indicate the the call has crossed a VOIP gateway. I don't know if the NANP can handle assigning unique numbers to all VOIP gateways. Other PTSN numbering plans in other parts of the world may also be running short of numbers.
Just another reason to push the development of security protocols for VOIP. Here we go again, security as an afterthought.
