The process has been outlined for many years, and it is not a simple one:
1) NEVER give your SSN to anyone that legally cannot require it (such as the IRS and banks).
2) NEVER use your SSN on public documents such as a driver's license!
3) Use an unlisted phone number.
4) Public services are available to prevent your address from being published (i.e. PO boxes).
5) Check your credit report regularly. Place a 'Fraud Alert' on them if you see any suspicious activity.
NOTE: Everyone can now get a free credit report annually.
6) When you print personal checks, never print the name on the checks the way you sign it (i.e. if you sign with initials, print your full name and vice versa) - this will prevent anyone from signing your checks the way you did on the signature card at the bank.
7) Keep copies of everying in your wallet in a safe place, and your passport if your're traveling.
