Here is another great windows based one (you need an XP SP1/SP2 cd to build it, not that hard to come by ;) http://www.ubcd4win.com/

One of the reasons I keep this one around is huge, you can load up ntuser.dat files and edit them. This means you can edit any part of the registry, including users, from a live cd. Everyone would be lying if they said they haven't gotten a little too zealous with the registry at one point or another.
