Ford is another prime example, they chose years ago that a 5% defect rate was more cost effective, while Toyota continued to strive for 0%. Look where they are now.
Unless US companies start making decisions based on what is best for the company, instead of how can I as the CEO get the biggest bonus, we all have a pretty grim future lurking ahead.
