If the trojans were activated here (said to be office documents), then it would seem that human ignorance was to blame.
I would take a guess to say the trojans were harvesters programmed in Office macro's. I've seen it done before but not in a very long time. Although, it obviously still gets the job done.
As long as there are people stupid enough to open the suspect emails then there is no hope. After all, the easiest way into a system is to capitalise on human stupidity.
